Peripheral smear

Normocytic normochromic red cells. Neutrophilic leucocytosis with toxic granulation. Platelets adequate, with clumps; count done manually.

Comment

Picture suggests bacterial infection. Correlate clinically. Blood culture reported separately.

Amendment

Version 3 replaces version 2, issued 2083-06-12 09:10. Platelets changed from 98 to 182 ×10³/µL after a manual recount. Reason: clumps on the smear gave a false low count. Amended by Dr. Anjana Shrestha at 11:32. Ward informed by phone at 11:35.
